Occupational Therapist – Rehab – (OT – Rehab)
StartDate: ASAP Available Shifts: 8/10/12 D Pay Rate: $1400.00 – 1950.00

Education: Certified by National Board of Certification in Occupational Therapy. Active and unrestricted state license as an Occupational Therapist.
Experience: One (1) year of direct work experience with hand and upper extremity, TBI, Stroke, and ability to co-treat with other disciplines (physical therapy, Speech).

LICENSE: AZ license
