StartDate: ASAP Pay Rate: $1400.00 – 1950.00

Required Qualifications
COVID VAC REQ
AVH Religious/Medical Exemptions forms will be reviewed by AVH – declinations not accepted
2 years of experience as an Occupational Therapist
Current Colorado Occupational Therapy license required
Current BLS certification through American Heart Association required

Preferred Qualifications
IMPORTANT NOTE: Traveler will be responsible for securing their own housing for the duration of this assignment.
MUST be healthy enough to safely function in a low oxygen
environment (8,000′ in elevation)
