Physical Therapist – Home Health – (PT – HH)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $1600.00 – 2100.00

Certifications needed for submission! 2+ references are required within last 2 years of work.
1.5+ acute nursing care (hospital base) and Home Care experience
Valid Driver License required for this need
Must be able to work independently with minimal supervision/assistance
Acute care experience preferred
Certified home health experience including proficiency with OASIS required and NON-NEGOTIABLE
AT LEAST TWO SPECIFIC INTERVIEW DATES/TIMES OF AVAILABILITY FOR INTERVIEW MUST BE ADDED TO TRIO/ PROFILE – PROFILES WILL NOT BE PRESENTED WITHOUT THIS INFORMATION. “ANYTIME” WILL NOT BE ACCEPTED.
