Pay Rate: $1650.00 – $2100.00

PT needed ASAP for 13 weeks
To carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize the patient’s progress toward achieving functional goals. To maintain a positive working relationship with other health professionals and ancillary staff .
Bachelor’s degree in Physical Therapy from an accredited program by the American Physical Therapy Association (APTA} Commission on Accreditation in Physical Therapy Education
[Wisconsin, United States] Physical Therapist
Current Physical Therapist license to practice in the state

Required Qualifications
